Applying for the NDIS
Australian Citizens and permanent residents aged 65 years and under with a permanent disability which has impacted on their daily life function may be considered eligible for access to the scheme.
To access the scheme
- Contact NDIA on 1800 800 110
- Request an ‘Access Request Form’
- Return the form to NAT@nedia.gov.au

NDIS Plan Review
The NDIA will contact you towards the end of your plan to arrange a review meeting. This meeting will provide the opportunity for you to review your existing supports goals and provide feedback where needed. Depending on your choice, a plan review can be done over the phone or in a face-to-face meeting.
You are welcome to bring along, or include in the call, a family member, friend, advocate, or other person when your plan is reassessed.
